In the event of a life-threatening medical emergency, call 911 immediately. DO NOT CALL THE OFFICE.
We are participating provider for Medicare and Medicaid. We participate in most major HMO and PPO plans. You are asked to bring your insurance card to EACH appointment. We ask you to provide us any information regarding changes in your insurance coverage, address or phone number.
ALL PATIENTS ARE EXPECTED TO PAY YOUR CO-PAYMENTS, CO-INSURANCE OR DEDUCTIBLE AT THE TIME OF SERVICE.
We have adopted the the "NO Show Policy" effective October 17, 2017. This policy states that we will charge patients $20.00 for each missed/no show appointment.
If you are unable to keep your appointment, please give us 48 hours or 2 days in advance notice to ensure that you will not be charged and we can give that time slot to somebody else.
We accept cash, checks, money order and all major credits cards.
Prescription refills will be processed within 2 business days (no weekends or holidays) of receipt of request. Refills received on Fridays will be processed the following week and will be completed by Tuesday.
If your prescription bottles indicates you have refills left, you DO NOT need to contact our office. Please call you pharmacy.
We DO NOT call in narcotics, others controlled substance and sleep aids.
Antibiotics will not be called in as an office visit is required. We reserve the right not to treat or diagnose patients over the phone without the benefit of the medical exam.
Please bring your medicines with you to EACH appointment.
We are a smoke free and gun free environment. Smoking and carrying concealed weapon is strictly prohibited in our premises.
